drivers side spring plate the 2 holes for the bolts are closer that what a u bolt would be.

on the brake lines depends on the callipers. older ones standerd size banjo bolt. and newer callipers are metric, witch is smaller banjo bolt that will work with 1/2 ton and 3/4 brake line.

Top plates are what the ubolts go into on top of springs

Yeah just a 4 inch. And top plates? Sorry I don't know much about drive train work.

Looked up these lift blocks, what the purpose of that middle insert and what is it made out of?

It puts pressure on the kingpin spring.  Steel

I can't seem to find a sight that has that lift block in a 4 inch, anybody know of one?

I have a 3inch on mine with 6 to 8 in lift and its good.  Don't want bigger then that imo
